When you finish the application, you’ll need to login through the parent portal (link below) to complete the forms. Once logged in please go to the forms and documents tab and fill out the paperwork that is there for your participant. Online signatures are available for all paperwork; however you will need to upload a current physical. To be current, the physical needs to be dated within the last 2 years. If it is at the end of the 2 years, a new physical is required. Physicals must be dated and signed by your physician.

All activity payments can be made through Camp Intouch. If a participant has a separate payee please give the Program Coordinator that information and they can add that to their account after it’s created. Along with Case Manager name and phone number if applicable!
